Position Summary:

We are seeking a skilled and compassionate Hospitalist to join our team and provide exceptional medical care to our senior patient population. As a licensed Physician, you will primarily work in hospitals, attending to high-risk and frail patients. In addition, you may occasionally conduct home visits and visit skilled nursing facilities (SNFs).

 

General Duties/Responsibilities (May include but are not limited to):

– Provide attentive and comprehensive care to patients, focusing on the senior population, primarily in hospital settings.

– Collaborate with covering hospitalists and specialty teams, ensuring effective communication and coordination of patient care.

– Work closely with the hospital and care managers to facilitate smooth transitions from the hospital to post-acute care settings.

– Collaborate with other clinicians and Case Managers to ensure daily assessments and care for all patients.

– Engage in regular conference calls with Case Managers to review patient cases and discuss discharge plans.

– Develop comprehensive and coordinated care plans, incorporating information from medical records, patients, families, primary care physicians, and specialists as needed.

– Maintain timely completion of medical records, including clinic visit notes and History & Physicals for inpatient admissions.

– Facilitate communication with patients, families, caregivers, and Primary Care Physicians regarding the plan of care and any changes in the patient’s health status.

– Work alongside the Case Management Team to provide support during care transitions between different settings.

– Actively participate in weekly complex case management multidisciplinary meetings.

– Offer recommendations to enhance the care delivery system to AHC leadership.

– Perform other administrative duties as required.

Minimum Experience:

– Preferred experience of 2-3 years in Hospitalist, Family Practice, or Internal Medicine. Graduating residents and fellows will also be considered.

Education/Licensure:

– Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.) with completion of an Internal Medicine or Family Medicine Residency.

– Active Medical License in the state of application, along with a DEA license. Board certification or eligibility in the respective specialty is required.

Other: – A career interest in management is preferred.
