Job Title: Nurse Practitioner

Location: 3364 Beyer Boulevard, San Ysidro, CA, US – 92173

Duration: Full-time and Direct Hire

Pay Range:  $50-$75/hour 

Position Summary:

Under the direction of the PACE Medical Director, the PACE Nurse Practitioner provides comprehensive medical care to the PACE participant. The PACE NP attends to the acute needs of the PACE participant, while managing the chronic medical problems, and paying attention to the preventive and rehabilitative care required by the PACE participant. The PACE NP incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the PACE participant.

Essential Functions of the Job:

1. Is responsible for prescribing appropriate medication, health regimens, and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues, and promote a healthier life.

2. Is comfortable managing stable patients with chronic conditions by utilizing internal resources, such as e-consult services, and other references to maintain their general health without the need/urge to refer patients to outside specialty care providers.

3. Is able to triage and quickly recognize when treatment is appropriately provided on site versus in the urgent care or emergency room.

4. Responds to and provides crisis intervention services when needed; ensures behavioral health services are delivered in a culturally appropriate fashion, and is consistent with State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.

5. Refers the appropriate patient to the appropriate Behavioral Health providers including: the Psychiatrist; the Psychiatric PA; the Psychiatric NP; the Doctorate in Psychology; the Licensed MFT team; and other appropriate BH members providing these services.

6. Efficiently and effectively prioritizes daily tasks and duties that help maintain high-acuity patients out of emergency and urgent care facilities and/or enables safe and expedient transitions of care.

7. Completes all required medical documentation in the Electronic Medical Record in a timely manner consistent with excellent documentation in compliance with San Diego PACE requirements, and State and Federal regulations

8. Assists in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services and quality assurance

10.Provides inpatient coverage and call coverage as assigned by Medical Director.

11.Screens and monitors the management of chronic conditions, symptomatic and well patients.

12.Coordinates the preparation of reports and written documentation including medical records, as required by the department clinic, state and federal agencies.

13.Supervises Nurses and assistants in examining room and may serve as preceptor for team members.

Other Responsibilities

1 Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

2. Enhances professional growth and development through participation in educational programs, current literature, in service meetings and workshops.

3. Attends meetings as required and participates on committees as directed.

4. Performs other related duties as assigned or requested.

Job Requirements

Education Required (Minimum level of education):
Minimum: Nurse Practitioner License, or Licensed Independent Practitioner
Preferred: Nationally Certified Family Nurse Practitioner.

Certifications/Licenses Required:
Minimum: RN license and CPR Certification
Preferred: MSN Preferred

Experience Required (Minimum level of experience):
Minimum: Demonstrated experience in community clinic and/or Managed Care System.
Experience working with the elderly.

Supervisory Experience Supervision of nurses.

Equipment Used: Diagnostic and therapeutic spirometry (e.g. EKG)

Verbal and Written Skills Required to Perform the Job: Bilingual (English/Spanish) Preferred. Good oral and written communication

Technical Knowledge and Skills Required to Perform the Job: Wound Care, ACLS

Working Conditions and Physical Requirements: Prolonged periods of standing and
walking. Frequent sitting. May assist patients in getting onto examination table. Will be
required to work at other sites. Will be requires to work participate on on-call panel and
work evenings and/or weekends.

Equipment Used: Computers, phones, personal vehicles, copy machines and fax machines
and other office equipment.

Universal Requirements

Pre-employment requirements include I-9, physical, positive background and reference check
results, complete application, new hire orientation, and pre-employment PPDs.

Maintain privacy of all patient, employee and volunteer information and access such information
only on a need-to-know basis for business purposes.

Comply with all regulations regarding corporate integrity and security obligations. Report
unethical, fraudulent, or unlawful behavior or activity.

OSHA Compliance
Comply with all regulations and required schedules regarding OSHA training.

PPD Compliance
Required health screening to be performed annually.

Policies and Procedures Compliance
Responsible for following all SYHC Inc. policies and procedures.
